About this patient intake process flowchart

Intake sets the tone of a visit and drives a lot of front-desk work. Moving forms and insurance checks before the appointment shortens check-in and reduces surprises at checkout.

This example is a general outpatient clinic flow. It is not clinical or legal guidance; privacy rules such as HIPAA apply to how intake data is collected and stored.

Step by step

  1. Schedule. Book the visit and send forms ahead of time.
  2. Verify. Check insurance eligibility before the visit; discuss options if it fails.
  3. Check in. Confirm ID and review the forms.
  4. Clinical intake. A nurse records vitals and history before the provider sees the patient.
  5. Checkout and follow-up. Book follow-ups and share the visit summary.

How to make it in flow-chart.io

  1. Start from the example. Edit the text in the generator box above so it names your own clinic steps and systems, then press Generate. The free preview needs no sign-up.
  2. Add the branches. Add the decision points that matter for you, for example new vs. returning patient? or telehealth visit. Each decision becomes a diamond with labeled outcomes.
  3. Assign owners. Save the diagram to the editor (free account) and rename steps to show who does what: front desk, nurse, provider and billing.
  4. Share or export. Share a read-only view link: people with the link can view the diagram but not edit it. Downloads as PNG, SVG or PDF are part of Flow Pro (see pricing).
